need a copy of my CITI Completion Report/Certificate. How can I get one?

I
just moved from another institution to MGH/BWH/FH. Can
I transfer my CITI training here?
Yes

Log in to the CITI
site
- Click
on “remove my affiliation from (Name of your old institution)
then follow the instructions for choosing your new
institutional affiliation.
After
you have updated your “affiliation” online and contacted
the Partners CITI Administrator
here, your name and CITI certification date will be
added to Partners Human Research Committee (PHRC) education
training list.
I
completed the CITI course for BWH but I am also affiliated
with another Partners institution. Do I need to complete
the same CITI course for each institution?
No,
once you complete the CITI course for BWH/MGH/McLean/Spaulding,
your CITI completion record is received via email and
recorded on PHRC education training list. If you completed
the course while affiliated with any other institution,
you need to follow the directions above for choosing
your new institutional affiliation.
Are
Other Programs Accepted?
The Boston area academic medical centers will accept
one another’s training requirements. All other training
requirements will be considered on a case by case basis.
How
will I know when it’s time to take the Refresher Course?
You
will receive an auto-generated email notification with
Subject line: CITI Program email reminder from
CITI Office 30 days and again 7 days prior to your anniversary
date.
How
do I find out if someone has completed the required
CITI training?
The current CITI status of study staff will display directly on the eIRB Staff & Access page in Insight. The staff may also login directly to the CITI web site and view their completion date (see first question above), or you can contact the Partners CITI administrator for more information.
How
will I know when my CITI training date has expired?
You will receive an auto-generated email notification with Subject line: [persons' name] CITI Completion Report Will Expire Soon from CITI office 1 day after your CITI training date has expired.
