Ensure Patient Assist Program

    Description:

    The Abbott Patient Assistance Foundation is designed to improve lives by supplementing medically essential nutrition products to financially disadvantaged individuals whose existing resources limit access.

    Brands include Ensure, Glucerna, Similac, PediaSure. See full list.


    Benefits:

    All applications are reviewed on a case-by-case basis. Participants will receive supplementary nutritional beverages delivered to their homes. Patients are eligible for a delivery of 6 cases twice a year.

    Eligibility:

    The patient must:

    • Be a U.S. Resident
    • Meet income guidelines (under 300% FPL, though they prefer this not be shared with patients)
    • Be totally reliant on the product meaning 100% of daily calories must come from the requested product
    • Not have access to any other form of payment such as insurance coverage

    Product can be taken either orally or via a tube-feed, but generally to meet the 100% of daily calories requirement it will be administered via a tube-feed.

    Applications may be faxed in with patient's proof of income.

    The doctor must fill out a section and sign the application. The patient must fill out a section, sign the application and attach proof of income, and any denial letters from insurance companies. Both the patient and doctor are notified of acceptance into the program. The estimated timeline is 3-5 business days for approval and then the nutritional supplement is shipped within 5-7 business days.

    The doctor/doctor’s office must contact the company to arrange refills. Every year a new application is needed.
