- Step One: call oral report to Child-at-Risk Hotline at 800-792-5200. Ask for address of appropriate DCF office to mail written report.

- You will need to complete a one time registration process, that will set you up with a user name/ID for future use. Recommended that you familiarize yourself with the process before you need to make an actual report.
- To report: Scroll to “How to Report,” select “Online” and then continue by selecting “First Time Submitting an Online Written Abuse/Neglect Report.” This is where you will complete the one-time registration process that will set you up with a user name/ID for future use.
- Once you have a username and password, you will log into the “virtual gateway” by selecting “Already Submitted an Online Abuse/Neglect Written Report” and logging in.
- Select “DCF Partner Services” to complete your online report and also view past reports made through your virtual gateway dashboard.
- Once you have submitted the report online, you can select the PDF icon to save it to your H drive, email it as an attachment, or print your report.

- Do NOT use the online system if any of the following apply:
- Emergency or urgent situations that require immediate attention from Protective Services as it may take more than 24 hours to process online reports.
- You do not have the name and address of the victim. The online reporting system will not allow you to file such reports.

- In accordance with current CDC guidelines, patients and caregivers who acquire COVID-19 infection may return to the Hope Lodge under the following conditions (as of January 2022):
- At least 10 days have passed since symptom onset and
- At least 24 hours have passed since resolution of fever without the use of fever-reducing medications and
- Other symptoms have improved and
- Written documentation from a health care professional is provided stating that the patient/caregiver is able to return to the Hope Lodge and the above conditions have been met.
- In accordance with current CDC guidelines, patients and caregivers who acquire COVID-19 infection may return to the Hope Lodge under the following conditions (as of January 2022):

- Note: They require that each patient stays with a caregiver. Exceptions will be considered on a case by case basis by submitting the Caregiver Exemption form- must be completed by medical provider; social workers are acceptable medical providers. Generally brain tumor pts and transplant patients will not be granted caregiver exemptions.
- Length of Stay policy- maximum of 9 months in 12 month period.
- Shuttle/Lyft service (as of 8/22): In our Covid-impacted world since October 2021, the shuttle has been a driver in a rented sedan that guests sign up for at least the night before, according to when their appointments are. We also have a relationship with Lyft which allows us to call rides for guests if appointments are before 8am (when the shuttle driver starts), after 4pm (when the shuttle driver ends), or if the schedule is too full to accommodate another person. We have now been given the ok to allow more than one caregiver and guest in the shuttle at a time, so if it works out that we can consolidate, sometimes that happens.

- MassHealth provides transportation for MassHealth Standard, CommonHealth and CarePlus members to medical care within one's "locality" if family, friends and public transportation are not available or are inappropriate. Locality refers to medical providers within 25 miles of home. However, when necessary medical services are unavailable in the member's locality, transportation to the nearest medical facility in which treatment is available is covered by MassHealth. - Special accommodations requests include transport by sedan (rather than a van- perhaps because patient cannot climb into a van), patient needs to ride in front seat, or patient requires an exclusive ride (not a shared ride), etc. These other Special Accommodations must be requested in the online PT-1 application (list in ALL three of the free text fields). MassHealth may require a letter of medical necessity from the authorizing provider.

--Asks staff to call MassHealth taxi (MART) on patient's behalf - As of 11/13/18 hospitals, including MGH, will no longer have “delegated” status that allowed us to complete Level II screens when indicated for serious mental illness (SMI).
- All PASRR Level II screens must now be completed by Univeristy of Massachusetts Medical School (UMMS) (for SMI) and/or Department of Developmental Services (DDS) (for intellectual or developmental disability or related condition).
- If indicated, both screens will be required.
- Case management is responsible for facilitating this process.

- We’ve been advised that information available from the patient, records and any family statements is sufficient for completing the Level I screening form (that determines the need for exemptions, level II, etc.). It is not necessary to contact community collaterals solely to assess for recent interventions.
- Social Service should not be formally consulted for the OBRA/PASRR process, but may offer guidance if already following. Case managers may ask for a curbside consult.
- Exempted Hospital Discharge (30 Day rehab stay) - if eligible, a level II is NOT required (see Level I form, question 9)
- To qualify a pt must meet 4 criteria:
- Be admitted directly from an acute hospital after acute medical care
- Observation stays and ED visits do NOT qualify
- Need nursing facility (NF) services to treat the same condition treated in the hospital
- Example: pt will be finishing course of IV ABx for pneumonia at NF = being treated for the same condition.
- BUT if pt needs NF stay because deconditioned from pneumonia (has finished IVs), would NOT qualify for Exempted Hospital Discharge. However- may qualify for Convalescent Care Categorical Determination. This then would require an Abbreviated Level II phone screen- see next section for more information.
- Example: pt will be finishing course of IV ABx for pneumonia at NF = being treated for the same condition.
- Is not a current risk to self or others and behavioral symptoms are stable and
- Is expected to stay in the NF for less than 30 calendar days as certified by the hospital's attending or discharging practitioner
- Be admitted directly from an acute hospital after acute medical care
- To qualify a pt must meet 4 criteria:
- Advanced Dementia Exclusion (ADE) and Categorical Determinations (CD) - SMI only*
- Request an abbreviated Level II phone screen via fax cover sheet. If approved for ADE or CD pt does not need an in-person Level II
- Advanced Dementia Exclusion definition- see p 6 of Level I
- Categorical determinations: convalescent care, terminal illness, severe physical illness, provisional emergency, respite

- Response expectations - Abbreviated Level II phone screen: UMass Medical School (UMMS) has 5 business hours to return a completed ADE or CD determination. Office hours: 9-5, 365 days/year. (Remember this is not an option for ID/DD cases)
- If the patient is found not to meet criteria for ADE or CD, UMMS is to contact the referrer the next business day to schedule an in person Level II
- *The only exemption for ID/DD is Exempted Hospital Discharge (30 Day rehab stay). Advanced Dementia Exclusion and Categorical Determinations do not apply.
- Response time expectations- full Level II in-person screen: UMMS and/or DDS have 2 business days to come to the hospital to complete PASRR. They have up to an additional business day to complete the screen and share the determination notice.

For NH residents OUR STAFF DO NOT COMPLETE THE FULL LEVEL II. It requires a face-to-face screening conducted by contracted NH PASRR agency. Instructions: If the pt requires a Level II screen.

- Required for ALL New Hampshire residents seeking Nursing Facility Placement (unless a screen is already on file. If came from a SNF the SNF should have it, otherwise contact PASRR office).
- Detailed instructions on form- read carefully.
- Must be co-signed by MD, APRN or PA.
- Certain exemptions ("Categorical Group Determinations"- Section VI of the Level I screen) from Level II screening are not automatic- they must be approved by the PASRR office. Submit Level I screen with required additional documentation to PASRR ofc. and await determination and next steps.
- If after completing the Level I form NO Level II screen is required:
- Retain a copy of the Level I Screening form for the medical record and send a copy with the patient to the SNF.
- No need to submit to the PASRR office.

- Waiver of in-person appearance and 3 day waiting period to get a marriage license.
- Normally BOTH parties appear in-person to file a Marriage Intention form at the Registry (Boston City Hall) AND there is a mandatory 3 day waiting period before the license can be picked up.
- Both of the above may be waived when one party is seriously ill and hospitalized or one party is on Active Military Duty and not in MA (for Active Military please call the Registrar's office for guidance: 617-635-4175)
- Process to get a Marriage License- INSTRUCTIONS to request to waive in-person appearance and FORM - must be signed by MD or PA
